Patient Eligibility for Trust Assistance

We extend our assistance to all individuals in need, irrespective of religion, caste, community, gender, or geographical location. Whether Muslim, Hindu, Christian, Buddhist, Sikh, or belonging to any other faith, our mission is to serve every Indian with compassion, dignity, and equality.

1. Application Process

  1. 1 - Patient or authorized representative submits a Trust Assistance Application.
  2. 2 - Required documentation is collected, which may include:
  • A. Proof of identity (Aadhar Card, Ration Card)
  • B. Proof of income (Income Certificate and Salary Certificate)
  • C. Patient / Guardian Bank statement for 1 year
  • D. Electricity Bill
  • E. Estimation / Cost Letter from Hospital

2 - Assessment & Determination

  1. 1 - Staff conduct an objective review using the standardized Eligibility Assessment Checklist.
  2. 2 - Findings are documented in the patient record.
  3. 3 - A determination is made:
  • A. Approved
  • B. Conditionally Approved (pending additional information)
  • C. Denied
  1. 4 - If required, cases are escalated to a review committee or supervisor.

3. Patient Notification

  1. 1 - Patients are notified in writing of the decision.
  2. 2 - Notifications include:
  • A. Approval details and assistance scope, or
  • B. Reason for denial (in plain, respectful language), and
  • C. Instructions for appeal or reapplication, if applicable.
  1. 3 - Notifications are delivered within the established response timeframe.

4 - Documentation & Recordkeeping

  1. 1 - All application materials and decisions are securely stored.
  2. 2 - Records are maintained in compliance with privacy, confidentiality, and retention policies.
  3. 3 - Access to patient information is restricted to authorized personnel only.
